市科技局关于印发天津市《外国高端人才

确认函》经办流程的通知

为贯彻落实科技部有关要求，畅通外国高端人才来华通道，更好地为外国高端人才来津创新创业提供便利，特制定《天津市<外国高端人才确认函>经办流程》，现印发给你们，请遵照执行。

天津市《外国高端人才确认函》经办流程

为紧紧围绕我市加快科技创新进程，实现高质量发展的总体部署，进一步加大外国高端人才引进力度，在深入推进疫情防控工作的背景下，根据科技部有关工作部署，结合我市引智工作实际，经研究，制定《天津市<外国高端人才确认函>经办流程》。具体内容如下：

一、发放对象

《外国高端人才确认函》（以下简称《确认函》）的发放对象为符合《天津市外籍高层次人才认定标准》，与我市聘用单位签订聘用合同或达成合作意向，确属必须、不可替代，并经主管部门审批，同意其来津工作或合作的外国高端人才。

二、申请程序

《确认函》的申请及签发程序：

（一）单位申请

外国人聘用单位登录外国人来华工作管理服务系统（http://39.155.161.12/），在线提交申请《确认函》所需的申报材料，包括：

1.《外国高端人才确认函申请表》（此表需网上填写后生成，打印并签字盖章后上传至系统中）；

2.聘用单位邀请函件或聘用合同、合作协议；

3.证明被邀请人符合《天津市外籍高层次人才认定标准》的相关材料，主要包括任职证明、获奖证书、收入证明等；

4.被邀请人护照或国际旅行证件（首页）；

5.被邀请人6个月内正面免冠照片；

6.聘用单位防疫责任承诺书（见附件1）；

7.被邀请人对天津市相关防疫措施知情书（见附件2）。

（二）在线受理

市科技局引智育才工作处收到申报材料后2个工作日内，对申报材料进行审核。材料不齐全、内容不规范的，在线一次性告知需补正的材料。

（三）在线签发

经市科技局审核，符合条件的，在线向聘用单位出具《确认函》；不符合条件的，出具《不予受理通知书》，并说明理由。

（四）送达本人

聘用单位将《确认函》通过电子邮件发至外国高端人才本人。

三、办理签证

外国高端人才自《确认函》签发之日起6个月内，按我驻外机构的要求，持相关材料至我驻外使、领馆或我外交部驻香港、澳门特别行政区特派员公署，申办R字签证。材料包括：

（一）签证申请表；

（二）本人护照（有效期为6 个月以上）以及符合规定的照片；

（三）《外国高端人才确认函》打印件；

（四）驻外使、领馆等驻外机构要求出具的其他材料。

四、相关事项

（一）我市外国人聘用单位是履行本单位引进外国人疫情防控责任的主体，应严格落实中央及我市疫情防控要求和措施，按规定制定、报送并执行防疫责任承诺书、疫情防控工作方案和被邀请人相关防疫措施知情书等相关材料，统筹做好外国高端人才入境后的疫情防控和管理服务工作。

（二）市科技局将在认真落实安全责任的前提下，做好相关政策的培训与咨询工作，并严格落实市疫情防控指挥部的部署要求，及时将《确认函》经办信息通报市疫情防控指挥部外事组组长单位，积极配合做好外国人入境防疫工作。

（三）本《经办流程》自印发之日起实施，并将视疫情变化及国家相关政策进行及时调整。

附件1

聘用单位防疫责任承诺书

天津市科学技术局：

本单位填写单位名称 邀请填写外国人国籍外国人填写外国人姓名护照号（填写外国人护照号）来津开展填写来津理由，应理由充分，确属必要工作。被邀请人符合天津市外籍高层次人才认定标准中的外国高端人才条件，申请为其办理《外国高端人才确认函》。

本单位郑重承诺：本单位已建立疫情防控方案，严格遵守《中华人民共和国传染病防治法》等相关法律和天津市政府疫情防控的相关规定，有专门人员负责外国人入境防控工作，配合海关、口岸、卫生、公安等部门做好外国人入境、隔离、检测、动态跟踪、个人防护等相关工作。

附件2

被邀请人对天津市相关防疫措施知情书

为做好我市新冠肺炎疫情防控工作，保障您的身体健康和生命安全，在津期间请您遵守我国相关法律法规，配合相关防控措施，现就相关事宜告知如下：

一、我市对所有经口岸入境人员（含港澳台地区）实行健康医学观察和核酸检测全覆盖，相关费用由入境人员自理。

二、请主动告知在津聘用单位您入境中国前14天的旅居史和个人健康状况，拟入境的交通方式，以及航班号或火车、轮船班次等信息，配合聘用单位做好防控预案。

三、请每日进行体温检测，在公共区域请自觉佩戴口罩，不做人群聚集性活动，保持一定的社交间距，做好工作、生活环境的日常清洁，保持室内空气流通。

四、在津期间如出现身体不适就医时，请主动告知医生14天内的旅居史，接触人员等信息，配合做好流行病学调查。一旦确诊为新冠肺炎，请保持良好心态，加强与医护人员沟通，积极配合治疗。

五、请严格遵守《中华人民共和国传染病防治法》等相关法律和天津市政府疫情防控的相关规定。如有拒绝执行，甚至扰乱防疫秩序，危害公共健康和安全的行为，天津市有关部门将依法追究法律责任。

本人已认真阅读知情书及附件内容，并承诺在华期间遵守中国法律法规，配合相关防疫工作。

Letter of the Invitee’s Informed of Relevant Epidemic Prevention Measures of Tianjin

To do a good job in the COVID-19 prevention and control, and guarantee your physical health and life safety, you shall please, during your stay in Tianjin, observe China’s relevant laws and rules, and cooperate with relevant prevention and control measures. Now, you are informed of relevant matters as follows:

1. The City implements full-coverage medical health observation and nucleic acid detection on all personnel entering Chinese Mainland through ports (including personnel from Hong Kong, Macao and Taiwan) at their sole expenditure.

2. Please take the initiative to inform your employer in Tianjin of your residence history and personal health status fourteen (14) days before you enter China, the intended means of transportation to enter China, as well as the flight number or train, shipping shift, etc., and cooperate with the employer to make a the preplan for the epidemic prevention and control.

3. Please measure your body temperature every day, wear a mask consciously in public areas, avoid crowd gathering activities, maintain a certain social spacing, do a good job in daily cleaning of the working and living environments, and keep the indoor air circulation.

4. If you feel uncomfortable physically during your stay in Tianjin, please tell the doctor initiatively of your residence history within fourteen (14) days, and the persons you’ve contacted, and cooperate with the investigation of epidemiology. Once you are diagnosed to be infected with COVID-19, please maintain a good mental state, strengthen the communication with medical staff, and cooperate with the therapy actively.

5. Please strictly observe the Law of the People’s Republic of China on the Prevention and Treatment of Infectious Diseases and other laws, as well as relevant regulations of Tianjin Municipal Government on the epidemic prevention and control. If you refuse to implement the aforesaid laws and regulations, and even disrupt the order of epidemic prevention and endanger public health and safety, relevant department of Tianjin will investigate you into legal liabilities according to law.

6. If you encounter difficulties and need help during your stay in Tianjin, you may dial the 24h government service hotline 88908890 and the 24h psychological assistance hotline 88188858.

I have read the Informed Letter and the attachment in earnest, and I hereby undertake to observe China’s laws and rules and cooperate with relevant epidemic prevention work during my stay in China.
